TO OBTAIN YOUR TICKETS:

Complete the online payment form below.

OR

Mail a check (or cash) in the amount of $20 per number of tickets requested, made payable to “Saint Patrick High School” and addressed to Saint Patrick High School, 5900 W. Belmont, Chicago, IL 60634, attn: Marybeth Nowak.  Include your name, address, phone, e-mail and number of Split the Pot tickets requested.  We will complete the ticket information for you, enter your ticket(s) in the drawing, and return a copy of your completed raffle ticket to you via email.
