CERTIFICATE OF CHILD HEALTH EXAMINATION

New medical dated at least one year from the day of entrance into the 9th grade must include up to date record of all vaccines received.
This medical report satisfies the athletic physical needed to participate in all sports for the freshmen year.
VISION SCREENING ALSO NEEDED
All updated forms should be received by May 1st
The Dental exam is optional and a
Hearing test not required.
